認知症の定義2

  認知症は成人になってから起こる認知機能の障害です。例えば、認知症の人があることを何度も何度も繰り返し聞いてくることがありますが、認知症の人にとっては、繰り返し聞いていたことも記憶にまったくないのです。

 健康な人の「物忘れ」が、心理テストなどで年齢相応の記憶低下を超える所見の場合は、「認知症」に発展する可能性が高いといわれています。参考のために軽度の認知症について以下にまとめました。

 【軽度認知機能障害は、MCI(Mild Cognitive Impairment)と呼ばれています。認知症を発症する予備軍です。軽い認知症はありますが、日常生活には支障がありません。正常な状態と認知症の境界線にある人といえます。軽度認知機能障害は半数以上が、アルツハイマー認知症などに進行するといわれています。ただし、すべての軽度者が認知症になるわけではなく、正常に戻る場合もあるといわれています。下記が診断基準になります。

 ① 記憶低下の愁訴がある。

 ② 日常生活には支障はない。

 ③ 全般的に認知機能は正常である

 ④ 年齢に比較して記憶力の低下がある。

 ⑤ 認知症は認めない。CDR(Clinical Dementia Rating)スコアが0.5(認知症の疑い)

 ※ CDRスコアが0.5は、再軽度アルツハイマー認知症とされています。

 ※ CDR-J(日本版)は、臨床認知症評価の方法の一つです。家族と本人に6項目で構成された記憶、見当識、判断力と問題解決能力、社会適応、家族状況、介護状況について質問していく形で進行していきます。
